What is the price?, How long have you been doing this, Ask for references, ask to see some of their work, ask if they will be the actual person working for you on that day, and how long will they stay their to record your wedding day including the reception, also let him know what you expect.
References :
First and foremost, get samples, or look at their website to determine their overwhelming style. Is the photographer more traditional or photojournalistic? Traditional encompasses more "staged" shots, while a photojournalistic photographer will make the most of candids--that is, getting pics of you and everyone else when you're doing something, like telling the "story" of the day through photo.

Are they modern? What type of enhancements can you count on? Are they included? Usually, retouching is automatic these days, but be sure to ask. And if you want something special like a black and white photo, but the roses in the picture showing up their true red color, you might have extra for this. Ask about all special finishes for your package, and what you have to choose from.

Will he/she shoot mostly in color? Black and white? Both? Determined by the type of pictures you want.

Does the photographer use digital or traditional film?

What type of lighting equipment will be in use?

Are you meeting with the person who is actually shooting the wedding? If not, ask to meet the actual photographer. Also, ask about assistants: if he/she has one and how they will work on that day.

Ask if you can provide a list of "must have" shots. This is usually the case, and a good photographer will consider this.

If you want photos "off location" (i.e. some place other than the ceremony or reception venue), ask about any additional costs, or things to consider. For example, if you're shooting outdoors and it's going to be really bright that day, versus shooting in a dark reception hall. It's just always good to let them know so that they have the right equipment (but, usually, they do anyway).

Ask about time expected from shooting to production (i.e. when you get them back). Ask about the formats you'll have as well --- traditional portfolio, or digital options?

That's all I can think of.
References :
1. Be sure to see sample albums/videos. For the photos, make sure they show you a good selection of photos from the same wedding. Sometimes they just pick one good photo from this wedding, and one from this one, and one from that one. But, you want more than one good photo! So make sure you look at a sample wedding album from start to finish to make sure all aspects of the wedding were taken well. I made this mistake. I saw a few samples, and thought they looked great. But the photographer was only good at certain types of pictures.

2. Ask wedding photographer/ videographer if you would be able to provide a list of must-have wedding photos. Like if you want to make sure to get a photo with your grandma, you and your mom all together. This was a mistake I made with my photographer and when the photos came back there were very few with my family and lots and lots of his family. I should have given him a list of photos I really wanted taken.

3. They will probably also want to know the number of photos you want taken, etc. This will be for pricing. Make sure you do your research on pricing in your area so you will know what price sounds good.
References :
Always find out how many photographers will be assigned to your wedding. You simply MUST have more than 1, 2 are good, 3 are ideal.
Find out what package deals the photog offers. Many times if you have them do your engagement photos, they will be more reasonable about bridal portraits and wedding day photos. (create a package at a lower price) Very important too, is how much time do you have? Some photographers do not give you enough time coverage. Find one that doesn't schedule more than one wedding a day and that promises coverage from the very beginning of your day till the end of the reception.
And very important and sometimes overlooked, give your photographer a list of the things they MUST cover. ie., and elderly relative that you may not get to see too many more times....the bridal/dressing room for those candid pre-wedding shots, like touching moments between bride and father!
Best wishes!
References :
ask for samples. that is probably the most important thing. You want to know exactly what you're paying for.

Just as important is that you read the contract CAREFULLY. Don't be caught by any surprises. And don't sign anything you aren't 100% comfortable with.

Also, ask for references of past events they've done.
